Procedure
Submission of new applications to Agrahara Division of National Insurance Trust Fund along with the following documents.
Required documents
- Application for Agrahara electronic card
- Certified copies of the Birth Certificates of the member and his / her dependents
- Certified copy of the National Identity Card
- If married, a certified copy of the marriage certificate
- Certified copy of the pay slip for the previous month
- Certified copy of the letter of appointment
- Duly completed application for Gold or Silver schemes under newly proposed insurance scheme

Procedure
When claims are requested from Agrahara, the application for claims should be submitted to the Agrahara Division of the National Insurance Trust Fund along with the following documents
Required documents
- Claim form for charges of medical officers and hospital (Agrahara)
- Original or a certified copy of a diagnosis card issued by the hospital
- Receipts of deposits
- Receipts for payments
- Final bill issued by the hospital
- Detailed bill
