To submit the application duly perfected as per Public Administration Circular No 22/99(xxxii) to the Chief Secretary through the Head of Department and Secretary of the Ministry.

- Recommendation of the Head of the Department/ Secretary of the Ministry where the officer served
- Certified copy of the letter of appointment
- Copy of the letter of confirmation in the service
- Certified copy of the letter of promotion to the grade in which the officer becomes eligible for obtaining a permit
- Certified copy of the letter by which the approval was granted for retirement
Application duly perfected is sent to the Department of National Budget through the Ministry of Local Government and Provincial Councils along with the above documents
